Do I need an aromatase inhibitor on testosterone therapy?

Usually not. Aromatase inhibitors such as anastrozole are used selectively, where measured estradiol and symptoms together indicate it. Estradiol has necessary functions in men, including in bone density, lipids and libido, so over-suppression causes its own problems. Adjusting testosterone dose or interval is often considered before adding one.
